2012 Small Business Celebration Nomination Form

You may nominate multiple companies for the 2012 Small Business Celebration. Expand the additional nominee forms below to make up to five nominations. If you would like to make more than five nominations, please fill out multiple forms. Thank you!

Criteria:

Must be a for-profit business headquartered in the Chamber's five county service area: Antrim, Benzie, Grand Traverse, Kalkaska, Leelanau
Employ no more than 150 full time employees
A minimum of three years in business
A visible commitment to good business practices
Growth and/or sustainability in the number of employees and/or sales
A history of good employee relations
Service to the community
At time of application, must be a member of the Traverse City Area Chamber of Commerce
CEO/President will be available for all required events as noted in timeline
Cannot currently be in the appointed leadership position on the Chamber's Board of Directors, Small Business Celebration Program, or other directly related position, nor can they be a sponsor of the celebration. The judging committee will be the final arbitrator in any perceived or actual conflicts.
